Community Mediator

Our Community Mediators are integral to public safety, neighborhood health and wellness. They build community through consistent, intentional interactions with all community members.

Oak Street Community Cabins

Oak Street Community Cabins are 24/7 emergency shelters. OCC provides residents the opportunity to stabilize from crisis and focus on their permanent housing journey.

Housing Services

Our programs work together to help get people housed starting with a "Hello" on the street by our Community Mediators to acquiring stable housing, supported by our Oak Street Community Cabin staff.

Community Services

Working with the community, the State of California campaign to Stop The Hate and C.R.I.S.E.S we work on building and healing the community through bias education, empathy walks and community events. Through our community engagement we enable our vision of going from 'Me to We'

Winter 2018

March 2019

Family Bridges emerged from the Civil Rights Movement. We provided outreach, advocacy and translation services.

The Community Health Home program was established under contract with the Alameda Alliance for Health and later in 2019 with Anthem Blue Cross, we supported high-risk plan members to improve their health. By providing nurse navigators and community health workers who conducted enhanced care management to prevent avoidable - hospitalization and emergency room visits.

Pilot outreach project with limited resources was launched. This included one volunteer who was formally incarcerated developing trusting relationships and distributing 30 meals a day to the houseless.

We offer to sites. Both offer a safe and welcoming environment for seniors to strengthen their social networks, engage in stimulating activities and increase their capacity to live independently.

Friendly Visitor

Learn More

Friendly Visitors provides companionship to homebound, socially-isolated Cantonese or Mandarin speaking seniors in North Alameda County . Program volunteers become an important link to the outside world for the isolated seniors.

Hong Fook

We help frail elderly and adults with disabilities regain their ability to live independently and ease the need for family care by providing health services and therapeutic activities in a safe and supportive environment.

Lake Merritt Child Care Center provides a warm, safe and, nurturing environment for the children trusted to our care and to provide a much needed service for working parents with preschool-age children.
